If you do not agree to these Terms, please do not use the Site or our services.
You agree to use the Site only for lawful purposes. You may not: use the Site in any way that violates applicable law; attempt to gain unauthorized access to our systems or data; copy, scrape, reproduce, or distribute Site content without permission; or interfere with the Site's operation or security. Appointments may be scheduled online, by phone, or in person. We require at least [24 hours] notice to cancel or reschedule an appointment. Late cancellations or missed appointments ("no-shows") may be subject to a fee of [$XX]. Repeated no-shows may result in changes to our scheduling policy for your account. Arriving late to an appointment may result in a shortened session to accommodate the next scheduled patient.

If we bill your insurance on your behalf, coverage and reimbursement are not guaranteed, and you remain responsible for any portion of charges not covered by your insurance plan, including deductibles, copays, and coinsurance. To help ensure safe and effective care, you agree to: provide accurate and complete information about your health history and current condition; inform your treating clinician of any changes in your condition; follow your prescribed treatment plan and home exercise program; and communicate any concerns or adverse reactions to your care team promptly.
If you receive services via telehealth, you acknowledge that virtual visits have inherent limitations compared to in-person care, that a stable internet connection and compatible device are required, and that telehealth is not appropriate for medical emergencies. Additional telehealth consent may be required prior to your first virtual visit.
MINT PT is committed to protecting your personal and health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. For details on how we collect, use, and protect your information.
If you opt in to receive text messages from us, that program is governed by our separate SMS Terms & Conditions.
Any patient testimonials or reviews shared on the Site reflect individual experiences and results, which vary from person to person. Testimonials are not a guarantee or prediction of the outcome you will experience.
